SNF Quality and Utilization Program Facilitator

In this role, you will optimize the quality, efficiency and effectiveness of clinical care provided by Kaiser Permanente clinicians facility staff through the use of quality, utilization, care/case management and customer services skills and techniques. You will assure efficient and effective transitions of care to and from the SNF setting as well as optimize patient/family satisfaction with care received.

The Skilled Nursing Facility (SNF) Quality and Utilization Program Facilitator's primary responsibility is to optimize the quality, efficiency and effectiveness of clinical care provided by Kaiser Permanente clinicians and facility staff through the use of quality, utilization, case/care management and customer service skills and techniques. As well as optimize patient/family satisfaction with care received in the SNF setting and to assure efficient and effective transitions of care to and from the SNF setting.

 

Job Requirements

• Minimum 2 years of experience in quality and quality management or equivalent; 2 years as Home Health case manager in last 10 years; 2 years of adult medical surgical experience in last 10 years; 2 years of geriatric clinical practice in home health, hospice, hospital or long-term care setting in last 10 years

• Bachelor's degree in nursing or equivalent knowledge and experience

• RN license in Oregon upon hire. RN licensed in Washington within 2 months of hire

• CPHQ certification in Health Care Quality Improvement within 12 months of hire

• Ability to effectively work with a diverse population of patients, families, staff and providers

• Ability to collect, analyze and report statistically meaningful quality and utilization data

• Working knowledge of admission criteria for SNF, Home Health, Hospice and Hospital
