Job Requirements • Minimum 2 years of experience in quality and quality management or equivalent; 2 years as Home Health case manager in last 10 years; 2 years of adult medical surgical experience in last 10 years; 2 years of geriatric clinical practice in home health, hospice, hospital or long-term care setting in last 10 years
