Innovative Senior Care, part of Brookdale Senior Living, has an outstanding opportunity for a RN to serve as the Assistant Director of Home Health Professional Services of their home health agency located on site within their upscale, retirement communities in the Chicagoland area.

The Assistant Director of Home Health Professional Services is a registered professional nurse who is responsible for the oversight and coordination of the RN Case Managers as well as the operational support for the Director of Home Health Professional Services (DHHPS). The Assistant Director of Home Health Professional Services is responsible for the delivery of services across care teams, ensuring care is coordinated as ordered, complying with company standards as well as state and federal guidelines.
