Overview: The Nurse Practitioner provides general medical care and treatment to patients in medical facility such as clinic, health center, or public health agency, under direction of Physician. Performs physical examinations and preventive health measures within prescribed guidelines and instructions of Physician. Orders, interprets, and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs. Records physical findings, and formulates plan and prognosis, based on patient's condition. Discusses case with Physician and other health professionals to prepare comprehensive patient care plan. Submits health care plan and goals of individual patients for periodic review and evaluation by Physician. Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures. Refers patients to Physician for consultation or to specialized health resources for treatment. Performs diagnostic and therapeutic procedures to assist physician in diagnosing medical and problems and prescribing necessary treatment and services for quality patient care; records the normal and abnormal data for review by the physician. Takes patient histories, conducts physical examinations, and records the date in the patient's record; writes progress notes; prepares patient workups and summaries noting pertinent positive and negative findings; obtains psychosocial history relevant to patient care, and assumes responsibility for developing therapeutic relationships with patients. Orders and schedules laboratory studies and diagnostic procedures; explains necessity, preparation, nature and anticipated effects of scheduled diagnostic and therapeutic procedures to the patient. Carries out therapeutic procedures; monitors progress of patients, reports progress to supervising physician, and maintains record of each patient's progress, consulting with supervising physician when patient's progress does not meet anticipated and/or predetermined criteria. Demonstrates on‑going competence in the principles of and current development in palliative medicine. Ability to work collaboratively with hospice employees and volunteers as part of an IDT.
