Job Description: 
Outcomes Coordinator reports directly to assigned Outcome Support Team manager. The Outcomes Coordinator will code assigned assessments following ICD-9 coding guidelines and review assessments for OASIS inconsistencies following OASIS guidelines. Provide OASIS educational needs to clinicians.

Job Functions

Responsible for reviewing SOC, ROC, Recert OASIS for accuracy and inconsistencies and making subsequent recommendations to the assessing clinicians via telephone and make agreed upon changes in point of care system and document changes not agreed to by clinician

 

Responsible for accurately coding all assessments, discussing with assessing clinician via telephone making agreed upon changes in point of care system and document changes not agreed to by clinician

 

Document in point of care system recommendations that the assessing clinician did not agree upon

 

Responsible for turning in weekly report every Monday AM of agency trends on OASIS review form

 

Keeps abreast of new developments in Home Health coding and OASIS

 

Participate in on call rotation as assigned

 

Participate in transition call as scheduled; maintains weekly telephone contact with Branch manager thereafter

 

Responsible for reviewing patient assessments for skill, homebound criteria, medical necessity 

 

Job Requirements 

RN License
 
Experience Desired
1 year Home Care experience
 
Skill Requirements
COS-C Certification
HCS-D Certification
Must Obtain COS-C and HCS – D Certifications within first 90 days of employment if not already obtained
 
Skills Desired
Understand and code per ICD-9 coding guidelines
Understand CMS Conditions of Participation, OASIS guidelines
Effective Verbal and Communications skills
Good Interpersonal Relationships
