Job Description: 
Maxim Healthcare Services' local branch is currently seeking a Primary Care Nurse (PCN). This individual must be a Registered Nurse who will be directly responsible for clinical activities of the field staff. With team members, the PCN plans, implements and evaluates patients' Plans of Care for appropriateness to individual patients' needs. Plans for, and assists in, conducting team conferences. The PCN also shares in the responsibility of assessing clinical case management activities with the Supervisor of Clinical Services.
Essential Duties and Responsibilities:
  • Admits patients for service and development of the Patient Care Plan
  • Implements or delegates responsibility for twenty-four (24)-hour nursing service and Homemaker / Home Health Aide services
  • Coordinates services with the patient, family and others involved in the care plan
  • Regularly evaluates patient progress and acts promptly when any change in the patient's condition is noted or reported, and terminates care when goals of management are attained
  • Identifies patient and family needs for other home health services and referrals when appropriate
  • Participates in orientation, teaching, and supervision of other nursing and ancillary patient care staff
  • Determines aspects of the care plan for delegation to a Homemaker/Home Health Aide. Whenever any patient care activity, other than those activities within the scope of practice under Connecticut regulations, is delegated to a Homemaker/Home Health Aide, the patient's clinical record clearly supports that the Primary Care Nurse or designated professional staff member has:
  • Assessed all factors pertinent to the patient's safety including the competence of the Homemaker/Home Health Aide, an
  • Determined that this activity can be delegated safely to a Homemaker/Home Health Aide
  • Develops a written plan of care and instructions for Homemaker/Home Health Aide services
  • Arranges supervision of the Homemaker/Home Health Aide by other therapists, when necessary
  • Visits and completes assessments of assigned patients receiving Homemaker/Home Health Aide services as often as necessary based on the patient's condition, but not less frequently than every sixty (60) days. The sixty-day assessment shall be completed by a Registered Nurse, while the Homemaker/Home Health Aide is providing services the patient's home
  • Enforces and demonstrates 100% compliance with all Federal, State, and ACHC regulations, reimbursement criteria as well as Maxim policies and procedures at all times
  • Prepares clinical notes and progress notes on nursing services delivered
  • Reports changes in the patient's medical or mental condition to the attending physician and to the Supervisor of Clinical Services
  • Participates in the continuing review of service, and participation in the performance improvement program
  • Plans for and assists in conducting multidisciplinary meetings on a biweekly basis or as directed with the DOCS/SCS and/or SCS
  • Develops a written plan of care and instructions for Homemaker/Home Health Aide services
  • Assumes on call responsibilities, covering all patients as required

    Qualifications:
    Minimum Requirements:
  • Current State license as Registered Nurse, BSN preferred.
  • Minimum of two years in an acute care setting with additional experience in home care preferred.
  • Thorough, sound knowledge of State and Federal regulations, as well as working knowledge of ACHC.
  • Demonstrated leadership capabilities with ability to relate well to staff.
  • Detailed oriented, flexible, good organizational and time management skills.
  • Thorough, sound knowledge of coverage criteria for Home Health services to include Medicaid and other third party payer sources.
  • Current CPR Card.
  • Current health certificate.
  • Current state driver's license and automobile insurance.
