Brooks Home Care Advantage, an affiliate of Brooks Rehabilitation in Jacksonville, Florida, is in search of a Director of Quality and Performance Improvement for its home health operations. Brooks is Northeast Florida’s second largest home health agency with a growing patient census of more than 1,000. The Agency has consistently been recognized by OCS Home Care Elite as one of the top home health agencies in the US. The Director of Quality and Performance Improvement will oversee, direct and audit all quality, performance improvement and regulatory aspects of Brooks’ clinical programs and services.
The ideal candidate will be knowledgeable in home health or hospice and be a consummate team player committed to superior quality, compliance and outstanding patient care delivery. The individual must be highly analytical, data driven, and possess exceptional organizational skills. The position will report to the Director of Nursing.
