The LPN/LVN is responsible for the supervision of nursing personnel to deliver nursing care and within scope of practice coordinates care delivery, which will ensure that patient's needs are met in accordance with professional standards of practice through physician orders,center policies and procedures, and federal, state and local guidelines. One year prior nursing experience preferred. Job Specific Details: Heartland Hospice in Albuquerque is currently looking for an LPN to help support our Albuquerque based team of RNs and CNAs. This position will support our patients in their place of rsidence when continous care is needed . Positions will also assist the RNs with making visits to our patients in the patients' homes.
