Hospice Medical Social Worker Provide wide range of psychosocial services to terminally ill patients and their families, including regularly assessing the patient and family psychosocial needs, and Pre-Bereavement/anticipatory grief needs. Implements the Plan of Care provisions to meet their needs, which include: social service goals for alleviating problems, supportive counseling, problem solving, community referrals pre-bereavement and bereavement care. Regularly re-evaluates the patient�s and family�s psychosocial needs. Adheres to Company policies and procedures and regulatory compliance requirements and directives. Requirements � Minimum age requirement of 18. � Bachelor�s degree in Social Work, Master�s degree preferred. Masters degree from school accredited by the Council on Social Work Education required in states as specified. � Licensure and/or certification as required by the State. � Minimum of two year�s social service experience in a health care setting or social services agency. � Knowledge of and ability to obtain community resources (information and referral, state/federal financial programs and eligibility requirements, etc.). � Compliance with accepted professional standards and practices. � Understands Hospice philosophy, principles of death/dying. � Ability to work in an interdisciplinary setting. � Physical exam in accordance with state law requirements. � Valid driver�s license and an automobile that is insured in accordance with state and/or Odyssey requirements and is in good working order. � Satisfactory references from employers and/or professional peers. � Satisfactory criminal background check. � Compliance with accepted professional standards and practices. � Ability to work within an interdisciplinary setting. � Demonstrate excellent observation, judgment and communication skills. Ability to provide good written documentation in a timely manner. � Self-directed with the ability to work with little supervision. Odyssey Hospice is now a part of Gentiva Health Services.
