Serve as the lead coordinator of all patient admissions activity while holding accountability as the primary coordinator of all aspects associated with the admissions process. Communicate by telephone and coordinate Odyssey services with referral sources, RN/Case Managers, insurance companies, patients and families. Coordinate services and/or equipment with contracted vendors. Maintain communication with pending patients, their families, and referral sources and routinely follow the progress of prospective patients and those referrals not taken under care (NTUC). Effectively use the Company�s information system in processing all admission information as required. Requirements: Odyssey Hospice is now a part of Gentiva Health Services.
