FULL-TIME HOME HEALTH RN CASE MANAGER POSITION IN COLORADO SPRINGS! Innovative Senior Care, part of Brookdale Senior Living, has an outstanding opportunity for a full time Home Health RN Case Manager to provide services in our upscale living communities located in the Colorado Springs area as well as in the general community. Together with the other members of the healthcare team, you will provide a high level of nursing care to our residents. Location: Colorado Springs * Must be a Registered Nurse in good standing in the state of CO. * Must have 1 full recent year of experience in Medicare Home Health * Must be proficient in OASIS C and PPS. * Must have strong coding skills. * Solid ability to interact and build relationships with older adults. * Strong organizational and analytical skills. |
