Through interdisciplinary teams of physicians, nurses, home healthcare aides, social workers, spiritual and other counselors and volunteers, VistaCare provides hospice care primarily designed to reduce pain and enhance the quality of life of patients facing life-limiting illness. This hospice care is most commonly provided in the patient's home or other residence of choice, including hospitals, assisted living communities and nursing homes. IPU HHA/ C.N.A Functions as a para-professional member of the Hospice team supervised by a Patient Care Manager. Perform various services for a patient as necessary to meet the patient�s personal needs, to promote the patient�s comfort, and a safe environment. Observe patients and report/document these observations and the services provided. Manage related expenditures in a fiscally responsible manner in accordance with the Company�s budget.
