Contact: KGoers@brookdaleliving.com Innovative Senior Care, part of Brookdale Senior Living, has an outstanding opportunity for a RN to serve as the Director of Home Health Professional Services for their in-house home health agency located on site at their upscale retirement community in San Jose, CA. The ISC Director of Home Health Professional Services, DPS, is a registered nurse with a bachelor's degree and at least two years of recent home health management experience. The DPS is responsible for the business operations of the agency which includes planning, organizing, developing and directing the management and administration of the agency in accordance with federal, state, and local regulations, standards, and guidelines as well as agency policies and procedures. The DPS may also provide mentorship and guidance of clinical guidelines to the field RN Case Managers as needed. Performs all functions in a professional and ethical manner and collaborates with other members of the health care team to ensure the provision of quality patient care. Director of Nursing / Director of Home Health Professional Services - Registered Nurse Requirements: *Five (5) years experience as a professional nurse with two (2) years of current administrative/supervisory experience in a Medicare home health experience within a PPS environment. Certifications, Licenses, and other Special Requirements *Licensed as a Registered Nurse in the state(s) in which he/she will be providing services.
