FULL-TIME RN CASE MANAGER POSITION IN SCOTTS VALLEY PROVIDING HOME HEALTH WITHIN BEAUTIFUL, UPSCALE RETIREMENT COMMUNITIES AND IN THE GENERAL COMMUNITY! Innovative Senior Care, part of Brookdale Senior Living, has an outstanding opportunity for a full time Home Health RN Case Manager to provide services in our upscale living communities located in Scotts Valley & the surrounding area as well as in the general community. We are looking for professional and compassionate RN's with excellent communication skills to enhance the lives of our elderly residents. Together with the other members of the healthcare team, you will provide a high level of nursing care to our residents. Location: Oak Tree Villa in Scotts Valley * Must be a Registered Nurse in good standing in the state of CA. * Recent Home Health experience a must. * Must be proficient in Medicare, OASIS C and PPS. * Must have strong coding skills. * Solid ability to interact and build relationships with older adults. * Strong organizational and analytical skills. |
