As a Licensed Practical/Vocational Nurse, you will: *Develop and maintain a good working with inter-departmental personnel, as well as other departments within the facility, to assure coordination of care is appropriately maintained. *Provides care and emotional support to the actively dying patient. Demonstrates appropriate notification of hospice team upon death. *Reports changes in patient condition to the Team Leader and Patient Care Coordinator (PCC) as soon as practical. Communicates with health care providers in obtaining interventions, medical information, and coordinating patient care. *Perform all assigned duties in accordance with our established policies and procedures and as directed by supervisors. *Completes patient referral forms and processes information according to organization policies and procedures. Communicates with patient, caregiver, Team Leader, and PCC for schedule of skilled nursing visits. Qualifications *Graduation from an approved school of practical nursing and current licensure/certification in the state(s) of practice * One year home health, oncology, or related field nursing experience preferred. *Current CPR certification *Good organizational and communications skills *Ability to function with minimal supervision expect more from Emerald Coast Hospice We give you everything you need to make the most of your career, including skilled, collaborative associates; a team-oriented approach and the resources of a national company.
