FULL-TIME RN CASE MANAGER POSITION IN DENVER PROVIDING HOME HEALTH WITHIN BEAUTIFUL, UPSCALE RETIREMENT COMMUNITIES AND IN THE GENERAL COMMUNITY! Innovative Senior Care, part of Brookdale Senior Living, has an outstanding opportunity for a full time Home Health RN Case Manager to provide services in our upscale living communities located in Denver as well as in the general community. Location: Denver network of retirement communities & general community home health * Must be a Registered Nurse in good standing in the state of CO. * Recent Home Health experience a must. * Must be proficient in Medicare, OASIS C and PPS. * Must have strong coding skills. * Solid ability to interact and build relationships with older adults. * Strong organizational and analytical skills. |
