As a Private Duty LVN You Will:

 

JOB SUMMARY

Responsible for providing home health services consistent with the mission, vision and goals of the Agency.  LVN is a professional member of the home health team who performs planned purposeful activity designed to enhance the physical and psychological functioning of the patient in accordance with a written plan of care prescribed by a physician.

 

DUTIES AND RESPONSIBILITIES

  • Ensure quality and safe delivery of home health nursing services.
  • Assist the registered nurse or physician in performing specialized duties.
  • Communicate with RN Clinical Supervisor on a regular basis regarding changes in the patient's condition and/or needs.
  • Perform the nursing care plan procedures as directed by the registered nurse including nursing services, treatments and preventative, and rehabilitative procedures.
  • Perform procedures at a level of care standard within the medical community, scope of practice consistent with the Vocational Nursing Practice Act, and follows Agency's current policies and procedures.
  • Observe, record and report to the physician and/or clinical supervisor the patient's reaction to treatment, including drugs, and any changes in the patient's condition.
  • Demonstrate the ability to correctly use; direct the use of medical supplies and equipment; to utilize and access both direct and indirect resources to insure patient safety.
  • Instruct other health team personnel, including home health aides and the patient's family, regarding the nursing care needs and other related problems of the patient at home consistent with the Licensed Vocational Nurse Practice Act.
  • Assist the patient in learning appropriate self-care techniques as appropriate.
  • Cooperate with other agencies providing nursing or related services to provide continuity of are and to implement a comprehensive care plan.
  • Participate in orientation, in-service education, utilization review and team conferences, and performance improvement activities.
  • Participate in patient care conferences.
  • Prepare and submit, as directed, all case documentation and required payroll information according to Agency policy.
  • Provide services within the Agency's catchment area.
  • Assist with discharge planning as appropriate.
  • Acknowledge patient rights including privacy and confidentiality.
  • Consistently display a professional appearance and conduct in all settings.  Interpersonal and interagency relationships are to be positive and professional at all times.
  • Assume responsibility and display reliability for the completion of all tasks, duties, communications and actions.
  • Support the mission and goals of the Agency.
  • Maintain professional growth and development.
  • Participate in performance improvement activities.
  • Abide by Agency policies and procedures.
  • Accept other duties as assigned.

 

Minimum Requirements:

Current CA Professional License

CPR Card

TB TEst or X-Ray within the last 6 Months (or must have w/in 15 days of hire)

Physical Exam within the last 6 Months (or must have w/in 15 days of hire)

I-9 Acceptable Documents
